- Carle employees click ADD COUPON CODE and type in CarlePayroll (auto deduct from 2 pay periods).
- Click APPLY. Click NEXT. Under NOTE TO SELLER, type in Employee ID# to confirm payroll deduction payment. See picture below for example.
- A Carle hospital payroll form must be filled in & submitted by Tuesday, September 16th in order to complete the purchase and receive the merchandise. Form & information available at the bottom of this webpage.
- ***Payroll deduction can only pay for the merchandise. Shipping direct to doorstep must be paid with credit/debit card.

Payroll Forms & Pick Up Details
If using payroll deduction, a Hospital Payroll Deduction Form needs to be completed and turned in by Tuesday, September 16th to complete your online order. See steps below:

1) Print, fill in, scan & return (cc [email protected]).
2) Print and turn into office
3) Pick up payroll slip at office & turn in

Location: Main Campus Hospital
Center Building 2 near the Yellow Sun elevators.
***If someone needs one outside of those hours, the person can send an email to [email protected] and an electronic version can be provided for the employee to print, complete and return via email to our office

You will be contacted to pick up your order. Brandie will be reaching out to each person individually to set up a time for pick up.
